  • History & Service Overview:
    Community Care for Kids was implemented in 2002 as a fee-based service for children referred by Care Management Organizations in New Jersey. In 2004, the program was licensed as a treatment home provider and became Medicaid eligible. With the help of caring, trained caregivers in the community, the program provides therapeutic foster care that is individualized to the youth's specific needs.

  • Program Goal:
    Our goal is to enhance the youth's ability to live in a community/family setting. We work with the youth’s team, and our partners in the community, to help stabilize behaviors and reunite the youth with her or his family or successfully transition to independent living.

  • Target Population:
    The program targets emotionally/behaviorally challenged kids (ages 5 – 21) who are not able to live at home but who are capable of functioning in a home environment with extra support. They may or may not have been in a more restrictive residential placement previously but they are ready for life in the community. Referrals are accepted from NJ’s Department of Children and Families.

  • Services:
    - Intensive case management services that empower youth to grow, thrive, face challenges, and practice healthy behaviors;
    - Recruitment, screening, licensing, orienting, and training for competent, caring families in the community who are able and willing to offer their home to a child in need;
    - Caregiver monitoring and 24-hour support;
    - Individual and group therapy.
